If you live, work, or study in New York State you can use the Comorbidities & Underlying Conditions Personal Attestation Form to demonstrate you are eligible to receive the vaccine due to certain comorbidities and underlying conditions
New York State is currently in Phase 1a and Phase 1b of the vaccination program.

What Can I Expect When I Get to My Vaccination Appointment?
Please arrive at your vaccination appointment with your driver's license.
Please wear a short-sleeved shirt for ease of access to the vaccination site.
If you are receiving a vaccine as one of the prioritized groups (not part of the general population), please bring documentation that can be used as proof that you are in a priority group (employee ID card; a letter from an employer; pay stub)
